HANDBOOKS & FORMS

Below you will find all the manuals and forms referenced in your Belize Field Guide. While I don’t require them all, it is HIGHLY RECOMMENDED that you use them all, or some form of them for your team. The forms I DO REQUIRE you to complete and keep with you are: Medical consent, medical assessment, minor traveling form and Foursquare Child Protection Manual. I REQUIRE you to submit to me a Master Team List.

You will want to print one form out for each team member, including the team leader, and then I suggest creating a “master folder” to hold all your documents. I would make a “divider” for each team member.

*Your team will be working with children, and therefore I am asking (and it’s required by Foursquare) that all team members read through Child Protection Manual or watch the video on the linked website.
